Care and support in Solihull

What we can offer

We aim to help you to stay healthy and independent by offering a flexible and responsive service where appropriate.  We  strive to  enable you  to stay safe and in the place where you  feel most comfortable, usually your  own home.

Below is some of the services we can help you with.

Reablement

This is a short term intervention to help you recover after an illness or condition that has effected you, by giving you the skills and confidence to live at home independently.

Personal Budgets

A Personal Budget is the amount of money allocated to you to spend on your social care support needs. It can be spent in any way that you choose, so long as it meets the needs identified in your Support Plan and doesn't put you at risk. These will be agreed as part of your assessment process.

Home care

Home care is a care and support service provided in your home to help you with personal and other essential tasks. For example, this could be things like helping you to wash, dress and prepare meals.

Day care

This can assist you to take part in day time activities. You might enjoy lunch clubs, community groups and courses. Day respite can also be provided to offer short periods of respite in a range of different ways either in the home; or in day centres

Occupational therapy

If, as a result of illness, disability or frailty, you or someone you know has difficulties carrying out everyday activities at home, then Solihull's Occupational Therapy Service may be able to help. Our Occupational Therapy Service can provide equipment and adaptations to help keep you safe and independent in your home.

Assistive Technology/Telecare

Assistive technology and telecare are devices and systems that can allow you to perform tasks that they you may otherwise be unable to do, or increases the ease and safety with which tasks can be performed. For example alarms and alerts in case of an emergency or sensors that can detect movement or falls.

Wheelchair service

The wheelchair service provides a wide range of wheelchairs and associated equipment that can help you stay mobile. This includes things such as, a special seating and postural support service, detailed assessments of your needs, ongoing re-assessment and modifications to ensure that your wheelchair meets your needs. Alongside regular safety checks and a comprehensive repair, delivery and collection service.

Disability support

There are many services available throughout Solihull to help people with disabilities. Some can help to make life more comfortable while others can help you live more independently. Some services are provided in response to your individual needs, others like information and advice are open to all.

Residential and nursing care

If it is no longer possible for you to live at home, we can help you with finding/applying for supported housing or a more permanent care home.

Help Leaving Hospital

If you are admitted to hospital you and you think you may need support from Adult Social Care to enable you to remain independent at home or require support in looking at more supported accommodation, you can ask the hospital staff  to refer you to a social worker, or alternatively contact our One front door service on 0121 704 8007.
