Transition Planning Young Peoples Future

This publication is a guide for parents and carers of young people aged 13 to 19  who have a Statement of Special Educational Needs.

It will help you to make plans with your daughter or son ready for their move from school to adult life.  This move is called Transition Planning.

If your daughter or son does not have a Statement of Special Educational Needs, you can still receive help from Connexions by contacting the Connexions Personal Adviser in the school they attend, or at your nearest Connexions Centre.
