Adult Mental Health Social Work Teams

To respond to the practical, social and emotional problems that may arise as a result of mental illness or severe mental health difficulties. To provide or organise services to assist people to tackle these problems.

What does this service offer?

Many people face practical, social and emotional problems as a result of mental illness or severe emotional stress. Specialist Mental Health Social Workers are available to respond to these needs in a way where, subject to legal safeguards, you remain in control of what happens. Help available includes:

  • assessment of need including assessment under the Mental Health Act in respect of need for care or treatment
  • advice
  • assistance in attempting to resolve practical, social and emotional problems
  • provision or organisation of support to enable people to live as independently as possible
  • identification of and referral on to other appropriate services
  • providing an appropriately swift response where needed

How do we decide if you qualify for this service?

A Social Worker/Care Manager will meet with you to assess your social care needs and discuss with you what help we may be able to offer.

Services are available to people aged 18-65 years old. However, if you do not fit into this age group you may still receive help. Services are prioritised to those most severely affected by their problems or who are most at risk.

There is no charge for the service, although you may have to pay for some of the services that may subsequently be organised, but these will not be provided without your agreement.
