Home Support for People with Mental Health Problems
Information on support available for people with mental health problems.
What does this service offer?
It offers planned practical help and support for you in your own home, usually following a period in hospital, or to avoid the need to go into hospital. It also includes:
- Assistance with personal care
- Practical help with everyday tasks
- Encouragement and help in developing confidence
- Extensive support during a particularly difficult period
Home Care Assistants are available at short notice. They can make frequent visits to help you and your relatives through a difficult period.
How do we decide if you qualify for this service?
Your doctor, nurse, or social worker can organise support from one of the Adult Care teams below. They will visit you to assess you social care needs and discuss with you whether Home Support is appropriate for your needs. The hours available are prioritised to those assessed to be in most need. Home support can be provided seven days a week, from 8am to 6pm.

A Social Worker or Community Psychiatric Nurse (CPN) will assess your needs. They will discuss your assessment with you and your carer where appropriate.
