The Meadow Centre
About the Meadow Centre
At the Meadow Centre we provide multidisciplinary assessment advice and support for children and their families with significant and complex medical, developmental and/or social needs.
The service is organised into 3 teams
- The team for younger children up to 6 years of age, with complex medical and/or developmental needs including difficulties that may indicate the possibility of an Autism Spectrum Disorder.
- The team for older children aged 7 years to 18 years with difficulties that indicate the possibility of an Autistic Spectrum Disorder.
- The Children's Disability Team is a team of social workers working with severely disabled children up to 18 years old. This team provides additional family support, information, care packages, access to short breaks and child protection services.
Each child/young person and their family are individuals. The nature of assessment and work carried out will vary according to the child/young person's individual needs.
When planning a child or young person's assessment we work together as colleagues from Health, Education and Social Care to ensure that we have the most up-to-date information as possible for the child/young person's assessment. In addition we will collect a range of information from the child/young person's parents/carers. Parents are considered as partners throughout all involvement with the service. A school visit may be required as part of the assessment process. Families will be actively involved throughout the time that we will be working together.
Assessments are planned to clearly identify a child's strengths and difficulties. Some children may receive a diagnosis if appropriate. At the end of the assessment families will have clearer understanding of their child's needs and a plan will be agreed around appropriate next steps
