Consultation Request Form
After a Duty Call conversation, it may be suggested that the educational establishment or community worker complete the Consultation Request Form.
Guide to filling in the form
- The form should generally be completed for children and young people only when an educational psychologist (EP)has advised this during a Duty Call.
- We ask that the form is filled in as completely as possible. The information we request is needed by the panel to make informed decisions.
- The consultation request forms are considered during a weekly Educational Psychology Service (EPS) Panel meeting.
- Not all consultation forms will automatically trigger an EP visit to you or to the setting. All requests receive a written response.
- If a request for EP involvement is agreed, an EP will first make contact within 10 working days.
- If direct contact is negotiated, this will occur within 30 working days from the acceptance of the request.
Priority for casework in 2009-2010
Priority for EPS casework involvement in 2009-10 is allocated to:
- Children and young people at risk of exclusion from an educational setting.
- Children and young people with complex difficulties where there are significant barriers to learning.
- Children and young people with complex difficulties where there are significant emotional, social and behavioural needs.
- Children and young people in care or unaccompanied asylum seeker children.
- Children and young people involved in bullying.
- Children and young people experiencing anxiety related to school non-attendance or transferring to a new school.
- Children experiencing difficulty separating from parent/carers in going to school or nursery.
As part of our service to the community, our agreement with local Academies and Independent Schools is to provide consultation work for children and families living in Solihull.
