Project Request Form

Project Request Form - for group, organisation, community or strategic work

Solihull Educational Psychology Service (EPS) can provide project work, including intervention, training and research. This has the potential to benefit the whole community. The project request form is used to ask for EPS involvement.

Guide to filling in the form

  • The form should generally be completed only when an educational psychologist has advised this during a Duty Call.
  • We ask that the form is filled in as completely as possible. The information we request is needed by the EPS panel to make informed decisions. The project request form asks for information regarding the following:
    • Programme of activity – this includes an explanation of how the project fits into a delivery plan, such as a School Improvement Plan, Self-evaluation Framework, Service Development Plan, Children and Young People’s Plan.
    • Expected outputs and timescales – this describes the action to be carried out by the project team, time sequence of dates, resources required or provided by the project team.
    • How the project will be evaluated – how will the project develop skills, knowledge and understanding of community members to result in positive changes for children and young people? What types of information will be gathered and how?
    • Expected outcomes for children and families – what will be the measurable benefits associated with the project? How will wider community be informed of the outcomes?
  • Not all requests will automatically trigger involvement. All requests receive a written response.
  • If we agree involvement, an EP will first make contact within 10 working days.

Priorities for 2009-2010

Priority for EPS project involvement in 2009-10 is allocated to:

  • Anti-bullying policy, audit and pupil surveys.
  • School’s duty to promote Community Cohesion.
  • Nurturing schools.
  • Social and Emotional Aspects of Learning.
  • Staff emotional well-being.
  • Other projects (that can potentially benefit from the application of psychology).

As part of our service agreement with local Academies and Independent Schools, we can provide project work for an additional fee.
