Learning lessons locally

Whenever a child or young person who lives in the area dies or is seriously injured/harmed and abuse or neglect is known or suspected, Solihull Local Safeguarding Children Board (LSCB) is required to undertake a Serious Case Review.

The purpose of the review is to establish whether there are any lessons to be learnt about the way in which local professionals and organisations work together to safeguard and promote the welfare of children and young people and to identify what those lessons are, how they will be acted on and what needs to change.

In order to learn lessons whilst maintaining the child/young person's and family's privacy an anonymised report (for reviews initiated post 10th June 2010) and an Executive Summary of the case is produced.

These set out the key findings from the review that are relevant to local professionals and organisations.

An Action Plan is also devised which sets out what actions need to be taken in order to improve how local professionals and organisations work together to safeguard children and young people. The implementation of the Action Plan is then monitored and audited by Solihull LSCB.

Below you will find the link to an Executive Summary and Action Plan of a Serious Case Review carried out by Coventry LSCB. Solihull agencies contributed to the Serious Case Review because of their involvement with significant extneded family members of the child who was the subject of the review.

Those who work with children, young people and parents/carers will find it useful to reflect on the key findings.   In addition, the LSCB will run briefing sessions to disseminate the learning and also offers a Level 3 training course on lessons learnt from serious case reviews.
