Learning lessons locally
Welcome to the website for Solihull Local Safeguarding Children Board formerly known as Solihull Area Child Protection Committee (ACPC). Solihull LSCB aims to ensure local organisations work effectively together to safeguard Solihull's children and young people and protect them from harm.
Whenever a child or young person dies or is seriously injured/harmed and abuse or neglect is known or suspected, Solihull Local Safeguarding Children Board (LSCB) is required to undertake a Serious Case Review.
The purpose of the review is to establish whether there are any lessons to be learnt about the way in which local professionals and organisations work together to safeguard and promote the welfare of children and young people and to identify what those lessons are, how they will be acted on and what needs to change.
Further details on when and how a Serious Case Review is carried out can be found in the Safeguarding Procedures Chapter 8: Serious Case Reviews.
In order to learn lessons whilst maintaining the child/young person's and family's privacy an Executive Summary of the case is produced.
This sets out the key findings from the review that are relevant to local professionals and organisations.
An Action Plan is also produced which sets out what actions need to be taken in order to improve how local professionals and organisations work together to safeguard children and young people. The implementation of the Action Plan is then monitored and audited by the Serious Cases sub group on behalf of Solihull LSCB.
Below you find an Executive Summary and Action Plan approved by Solihull LSCB in May 2006. Those who work with children and young people may find it useful to reflect on the key findings.
