We are looking for Social Workers who are enthusiastic and passionate about changing the lives of people with care and support needs, and carers, through the promotion of wellbeing and choice, making a real difference whilst enabling Adults and young people to increase their independence and have fulfilling lives, free from harm and abuse.

We currently have vacancies in the following teams:
- Hospital Team – Permanent contract working 37 hours per week
- Reablement Team – Maternity cover until 30 September 2023 working 22.5 hours per week
You will be working as part of a fast paced, busy Hospital Social Work team. You will be required to complete long-term assessments and support planning after the person has left hospital. We work with all residents of Solihull over the age of 18 years who require social care to enable them to leave interim services following discharge. The team currently work on a 7-day contract, and you will be required to be part of a rota to work at weekends and Bank Holidays. The posts are with the Hospital Post Discharge Team. This is an exciting role which involves multi-disciplinary working across health and social care. There are also great new initiatives which are underway with this role, namely the ‘Home First’ projects.
Our Reablement Team is made up of Occupational Therapists and Occupational Therapy Assistants, Social Workers, Reablement Social Care Facilitators, and our CQC Registered provider arm with Team Supervisors and Reablement Support Workers. An Early Response Service is provided by Universal Care Services who have dedicated a team to work with us on Reablement Programmes. Reablement is offered short-term both to support a recovery from injury or illness and as a prevention service to avoid deterioration, loss of independence and or hospital admission. We are also developing an additional service to improve the quality of life and sustainability of people’s independence. Therefore, Person-Centred and Strengths-Based approaches are at the heart of everything we do. As a social worker in the team your role will be to support and guide Care Act Assessments, Safeguarding, MCA’s and setting Care and support arrangements.
