Some children and young people with autism may need additional support as they transition (or move) from one setting to another. A range of help is available to support you and your child or young person to prepare for a change.

Our work listed below may be accessed by parents at time of transitions when stresses/behaviours become more problematic due to the forthcoming changes.
- individual advice appointments to parent/carers to advise on autism specific issues
- range of training courses for parent/carers from one day autism awareness course to 6 session programmes including behaviour management courses.
Our Birmingham helpline is accessible to all across the West Midlands.
Special Educational Needs Co-ordinator (SENCO)
If your child is in nursery the first point of contact around any difficulties should be the nursery's named SENCO, who would help you liaise with the school SENCO.
Access Criteria
The SENCO would have a discussion with any parent of a child on the school roll around concerns.

The model the team follows includes:
- transition meetings between Autism Team Early Years named teacher to the Primary School Autism Team Named Teacher
- review Meeting with Setting/Parents/Early Years Named Teacher
- SASSI meetings on transition
- parent workshops on transition
- parent surgeries

Solihull SENDIAS
Solihull SENDIAS Service (Special Educational Needs and Disability Information Advice and Support) is available to support children and young people with SEND and their parents/carers.
The vision for the service is that children and young people with SEND, and their parents/carers, will be more aware of their entitlements, the options available and will be able to make informed decisions.
Information, advice and support for children and young people with SEND and their parents/carers will be impartial, free to access, accurate, confidential, comprehensive and easily accessible/understandable.
Access Criteria
Children and young people aged 0 to 25, who live in, or attend school in Solihull, or are registered with a Solihull CCG GP.

This team supports children and young people to access learning after they have received a diagnosis of Autism.
The model the team follows includes:
• transition meetings between Primary school Named Teacher- Secondary school Named Teacher
• review meetings with schools/ Parents/ Named Teacher
• depending on need - small group work on moving schools, 1: 1 sessions exploring specific topics, supporting visits to secondary schools
• providing resources / booklets for pupils and families to use to support transition
• monitoring ISP visits in the Autumn term to review their move to secondary school
• SASSI meetings on Transition
• parent workshops on transition
• parent surgeries
Access Criteria
The child should have a confirmed diagnosis of Autism and then access can be arranged through your child's SENCO or by telephoning the team

At a certain point a young person’s care will need to transfer from Children’s Services to Adult’s Services. The age at which a young person transfers varies from service to service, please talk to your current service provider(s) about the transition arrangements for your young person.
