Nature of work

The Hive (based within Park View)

The Hive provides day opportunities for people with a learning disability that provides them with stimulation, engagement, involvement, development, and holistic support.

Green Acres (based within Park View)

Green Acres is a service specifically designed for adults aged 18 years and over who have disabilities.  Green Acres provides physical, emotional and social support to service users, and respite for carers. 

The Maples (based within Park View)

The Maples provides daily, a safe and healthy environment for Older adults some of whom are collected by Local Authority transport to/from Centre from/to their own homes.

The Courtyard

The Courtyard provides day opportunities for people with a learning disability that provides them with stimulation, engagement, involvement, development, and holistic support.

Green Hill way extra care and Long View extra Care

Green Hill Way (GHW) extra care and Long View extra Care provides support for Solihull Community Housing tenants who live independently in their own premises within the building.

23a Pike Drive, Three Trees Community Centre

Provides day opportunities for people with a learning disability that provides them with stimulation, engagement, involvement, development and holistic support. 

Colebrook Centre

The Colebrook Centre provides daily, a safe and healthy environment for people with a learning disability and older adults some of whom are collected by Local Authority transport to/from Centre from/to their own homes.

Information will be used for a limited number of purposes and always in line with our responsibilities to:

• Enable us to understand your needs fully
• Provide a safe service that meets your care and support needs
• Organise appropriate care and support for you if required
• Help improve our services
• Plan for future provision of services

We process a variety of information relating to individuals including:

• Personal details (Name, Home Address, Contact Telephone Numbers, Email Addresses, Date of Birth, Gender, Ethnicity, Religion, Nationality, Sexual Orientation, Disability, Language NHS Number, Date of Death)
• Height and Weight
• Health conditions, disability, diagnosis
• GP details
• Lifestyle and social circumstances
• Financial details
• Family details
• Lifestyle and social circumstances
• Financial details
• Employment and education details
• Referral/Assessment Information

Who information is processed about

We process information about:

• Service users
• Your family,  carers or representatives of the person whose personal data we are processing

Who information may be shared with

Where necessary or required we share information with:

• Family, carers or representatives of the person whose personal data we are processing
• NHS, including your GP and hospitals
• Emergency services
• Social Care professionals
• Advocacy services
• Care providers, for example, care agencies or residential homes
   

How long we will keep your information

Information is kept whilst people are receiving support from Adult Care and Support Operational Teams, and for seven years after this.

Transfers Overseas

Information may be transferred overseas to service user’s family members or representatives.
