NHS continuing healthcare (NHS CHC)

Some adults aged 18 or over with long-term health needs qualify for NHS continuing healthcare (NHS CHC).

If you're an adult with long-term complex health needs, you may be eligible for health and social care arranged and funded solely by the NHS. This is known as NHS continuing healthcare (NHS CHC).

NHS continuing healthcare can be provided in a variety of settings outside of hospital, including at home or in a care home. 

NHS CHC is provided by NHS partners and not Solihull Council.

Eligibility

To be eligible for NHS continuing healthcare, you must be assessed by a team of healthcare professionals. They will look at all your care needs and relate them to:

  • what help you need
  • how complex your needs are
  • how intense your needs can be
  • how unpredictable they are, including any risks to your health if the right care is not provided at the right time

How is NHS continuing healthcare assessed?

For most people, the assessment process involves an initial screening carried out by a health or social care professional. If you meet the criteria, you will be referred to the NHS CHC team for a full assessment. 

The full assessment is usually carried out by a group of health and social care professionals who have been involved in your care. They are known as a multidisciplinary team (MDT). 

The assessment should take into account your views and the views of any carers you have. You should be given a copy of the decision documents, along with clear reasons for the decision.
