This page provides information about what happens after reablement and alternative support if reablement is not suitable. 

What happens after reablement  

Following reablement many people will be able to live their lives without any ongoing care and support services.   

If you need long-term support following reablement, a discussion will take place between you and the hospital social worker or Reablement team to decide what happens next. This may include completing an adult social care assessment and financial assessment.  

What happens if reablement is not right for me? 

If reablement is not right for you, we will:  

  • provide you with information and advice on what other services or support may be available 
  • discuss other services that you may have to pay for 
  • help you explore alternative ways to find funding for services or support   

You will always have an opportunity to give feedback about your assessment. If you disagree with the outcome, you can ask for this to be reviewed.
