Creating an emergency plan
To ensure you are prepared in emergencies, we recommend that you create an emergency plan for yourself and for the person you care for.
To create an emergency plan for the person you care for, contact:
- their social worker, or
- Carers Trust Solihull
If you choose to create your own emergency plan, you should consider:
- the personal details, such as the name, address and contact details, for the person you look after
- any family or friends you wish to be contacted
- any professionals, such as doctors and nurses, you wish to be contacted
- the phone number of your social services contact
- details of the medication the person you care for is taking
- details of the ongoing treatment and therapies they are using

Carers Identity Card
As part of your emergency plan, you can also request a Carers Identity Card from Carers Trust Solihull, providing you are registered with them. You can register whether you receive support from Solihull Council or not.
A Carers ID Card lets people know your name and that you are a carer even if you are unable to tell them yourself. It also lets you store your most important phone numbers.

- phone: 0121 788 1143

Message in a Bottle
Message in a Bottle helps emergency services personnel to save valuable time in identifying an individual very quickly and knowing if they have any allergies or take special medication.
The Message in a Bottle kit includes a form containing personal and medical information. This is placed in the green bottle which is stored in the fridge. Two stickers are provided: one for the fridge door and the other for the inside of the property’s front door. Emergency personnel, such as police and paramedics, then know to look in the fridge when they see the Message in a Bottle stickers.
For further information or to request a bottle you can:
- contact Angie Goldingay on 0345 833 9547
