Support planning

Overview

Once we understand your strengths, goals, and the areas where you need support, we will work with you to create a support plan that suits your needs. We will also need to confirm that you meet the National Eligibility Criteria. 

Your support plan will include: 

  • what is important to you
  • things you can do to live well and stay well
  • care and support you need from others
  • support you need to care for someone if you are a carer

We want your care and support plan to work well for you, so your involvement is really important. This approach focuses on your strengths and gives you choice and control over the care and support you receive. 

We’ll review your support plan with you regularly to ensure it still works for you. 

Support planning for carers

If you are a carer, your support plan will highlight your strengths and outline any support required. This might include services for:

  • the person you care for
  • you
  • both of you

We’ll review your plan regularly to make sure it continues to work well for you and the person you care for.
