Your reablement assessment
Your reablement assessment is a chance to discuss what you want to achieve and how we can support you to regain your independence.
Reablement may be suitable if you:
- live with a disability, have the ability to develop new skills and want to remain independent
- are recovering from illness or injury
- have recently been discharged from hospital
How to access support
You will be considered for a reablement assessment if you:
- are 18 or over
- live in Solihull, and
- receive support from our Early Response Service (ERS) or Home Discharge Service (HDS)
You may also be considered for our Enhanced Reablement Programme.

What to expect
Your reablement assessment will take place when you have returned home. Someone may come to visit you or they may phone you, depending on which is best for you.
If you have difficulty understanding or remembering information, or have difficulty communicating, you may want to have someone with you or use an advocate to help you with your assessment.
The assessment will look at how well you can do everyday tasks. For example:
- walking and moving around safely in your home
- looking after yourself by getting washed and dressed or using the toilet
- preparing and cooking food
- eating meals and drinking
- getting out and keeping active
What happens if reablement is not right for me?
If reablement is not right for you, we will:
- provide you with information and advice on what other services or community support may be available
- discuss other services that you may have to pay for
- help you explore alternative ways to find funding for services or support
You will always have an opportunity to give feedback about your assessment. If you disagree with the outcome, you can ask for this to be reviewed.
